भारत में फिलहाल डाइबिटिक फुट अल्सर से संबंधित समस्याओं से ग्रस्त लोगों की अनुमानित संख्या कितनी है?

डॉ. ओसवाल: फिलहाल भारत में 7.70 करोड़ लोग मधुमेह से ग्रस्त हैं. इनमें से लगभग 20 से 25% लोग डाइबिटिक फुट अल्सर से ग्रस्त हैं. 20% लोगों में यह बीमारी इतनी गंभीर होती है कि उनका पूरा पैर अथवा घुटने से नीचे तक का पैर काटने के अलावा और कोई विकल्प शेष नहीं रहता. दुर्घटना के अतिरिक्त अन्य कारणों से पैर काटने की सर्जरी वाले हर 10 में से 8 केसों के लिए डाइबिटीज ही जिम्मेदार होती है.

डॉ. ओसवाल: फिलहाल पुणे में डाइबिटीज के कुल मरीजों में से लगभग 20% लोग डाइबिटिक फुट अल्सर से ग्रस्त हैं. इनमें से 50% मरीजों में संक्रमण फैल चुका होता है और उन्हें छोटी-बड़ी सर्जरी करानी पड़ती है. दुर्भाग्य से संक्रमण से ग्रस्त मरीजों में से, लगभग 20% लोगों के पैर काटने की नौबत आ जाती है. फुट अल्सर से ग्रस्त पुरुषों एवं महिलाओं का अनुपात 2:1 है.

डॉ. ओसवाल: डाइबिटिक फुट अल्सर विकसित होने पर, उसके इलाज का मुख्य ध्येय यह होता है कि यह संक्रमण शरीर में अन्यत्र फैलने न पाए. इसके लिए पहले पैरों की नसों में होने बाले रक्तसंचार का परीक्षण किया जाता है. कभी-कभी पैरों के अंगूठे की हड्डी में टेढ़ापन आने के फलस्वरुप भी अल्सर हो जाता है. ऐसे में पैरों की अल्ट्रासोनोग्राफी कर रक्तसंचार की जाँच की जाती है और पैरों का एक्सरे कर, हड्डियों की स्थिति का परीक्षण किया जाता है. इंफेक्शन बहुत ज्यादा फैल गया हो तो गैंग्रीन न होने देने के लिए सर्जरी द्वारा अल्सर काट कर निकाल दिया जाता है. पैरों में हुए अल्सर के ठीक होने तक, उसकी नियमित स्वच्छता करनी पड़ती है. इस तरह के मरीजों के लिए खास तरह के जूते बनवाए जाते हैं. अल्सर पर दबाव न पड़ने तथा उसकी दशा और बिगड़ने से बचाने के लिए इन जूतों का लगातार प्रयोग आवश्यक होता है.
